I think that indeed visual puns should not be used much in writing,
(which limit the various reactions to it).
If you use them anyway, you have two choices : consider the visual pun is
a metaphor for the character current state of mind and doesn't
actually occur or consider the visual effect really happens and is a common
occurrence in the world.
In the first case (subjective), the only visible effect will be the
(distressed) state of mind
of the character. That way, since characters have the same visual references
they can see the same thing as was described before or a completely
misreading of the character. This is how I tend to see things in
fanfictions.
The second case (objective) is more cartoonesque and is IMHO harder to
convey
in writing without risking ridicule. I think this is the kind of
reaction you have difficulties
with. I'd say in this case that reactions should be less passionate,
more bored
than confronted to the same event in reality. In a world where ending as
a puddle
of goo occurs everytime time a bishonen take your hand and look in your
eyes, you're no
longer impressed (or rather distressed) by someone melting before you.
