Well, the Punisher certainly has.  I don't have the comic anymore, but it's
the same sort of rough&ready pain-filled execution.  Batman might not kill,
but he does commit some rather extreme psychological acts on the ciminals
he's after (He's also been reinvented a number of times, which is why I
specified the 'darker' batmans).  Pre-Xman Wolverine once tracked down a
team of mercinaries over a course of months/weeks, killing them one by one.
One he eviscerated and left for the others...  Even spider-man has had some
rough moments.  That's one of the themes of spiderman comics, the line
sometimes is rather thin between a white-hat and black-hat.  Of course,
Ghost-rider usually doesn't kill, but it's been said that death is easier
than what he does to them...

I don't recall a single one of your examples torturing an opponent to
death.  Ghost Rider comes closest.  Curious, incidentally that the
youma didn't distintigrate on death the way they normally do.
