Their names may be similar, but are they anything alike in terms of
personality? Also, you'd have to explain why Susan never displayed any
Sailor powers. (Actually, I think there are some major plausibility
issues with this even if Setsy isn't Susan, but I'll leave them for
someone more knowledgable about Sailor Moon to comment on.)

Maybe they aren't similar in terms of personality, but remember that Susan,
age indeterminate, but obviously not emotionally an adult, is in terms of
this setup a much "younger" version (whatever that means for time
travellers) of Setsuna, a fully mature individual. Who knows what difference
on your outlook a few runs through eternity will make?

Ditto for the Sailor powers. Here's one possibility. In the First Doctor
episodes the idea of Timelord regeneration is absent. (Remember that "canon"
in fiction is often post-facto justification for shortcuts that help the
series maintain a forced continuity.) So, presumably, Susan will regenerate
at some point. And the senshi are "reborn", too. Compatible spirits might
well seek out suitable hosts... and who better to incarnate the Sailor of
Time than an inidividual who herself has a round dozen of lifetimes to look
forward too, as well as the inherent Timelord affinity for temporal
manipulation? Of course, regeneration is not reincarnation. Setsuna/Susan
may not be an entirely reconciled cohabitation of souls...

Alternatively: Susan never seems in the time we know her to show any of the
potential of her Timelord heritage, either. She is effectively stranded in
the Dalek invasion of Earth (AD 2160) timeline, sans TARDIS. (Which is, if
I'm not mistaken, not the only or even the main Earth timeline in the Doctor
Who chronology.) Perhaps the opportunity of incarnating Pluto is offered to
her as her one chance to escape, and reclaim her own past. But you can never
go home again, as they say... "At what price X?" is always a good story
starter.
