Depends whether you consider the dubbed anime as canon or not. I the
anime episode "Pelvic fortune Telling", the doc's mom is repeatedly referred
to as "Mother Tofu", indicating that the family name is Tofu. On the other
hand, she calls the doc Tofu herself, so it's all up in the air, as far as I
know.
IIRC, her name is given in the anime as "Kin Ono", which would certainly
argue for Ono being the family name. She doesn't appear in the manga, and I
don't believe we are ever given another name for Tofu in the manga.
