[personal] name is Tofu, his last [family] name is Ono. Can anyone confirm
based on canon?

    Depends whether you consider the dubbed anime as canon or not. I the
anime episode "Pelvic fortune Telling", the doc's mom is repeatedly referred
to as "Mother Tofu", indicating that the family name is Tofu. On the other
hand, she calls the doc Tofu herself, so it's all up in the air, as far as I
know.
