Ash was on the hook. The fish was in his mouth. If I may quote myself, "A 
hook grabbed on to the back of his shirt and he was yanked out of the water."

To further my point...

In the episode "Bye Bye Butterfree," Misty hit Brock in the head with a 
mallet. She used it in other episodes, too.
