I STILL do not know HOW to get ahold of the form for sure, but I was
told to ask the Queen's Own fanclub.  Also You MIGHT want to try WRITING
or emailing her pubishers (Baen and DAW Books...) for more Information.
Most Authors (DW, Bujold, etc.) Just DON'T want to see it, so that they
CAN claim ingorance of the "so-called" copyright violation.  (I REALLY
Hope that DW isn't a member of this list... *sigh*)
