Okay, I need to make sure of which Rei is which. Of particular importance
is weather Rei 1 or Rei 2 was the one who Shinji fell on top of and
touched her breast (No it's not for a lemon).

After watching the series again these are my best two guesses (* are
those that are clearly correctly, due to direct statements):

Rei 1- Died at the hands of Ritsuko's mother
*Rei 2- Died after self-destructing when fighting the ring Angel
*Rei 3- Died in Third impact

-or- it might be this:

Rei 0?- Died at the hands of Ritsuko's mother
Rei 1- Died just before episode 1 (the eye patch seems to be consistent
with her 'rebirth' in ep 23)
*Rei 2- Died after self-destructing when fighting the ring Angel
*Rei 3- Died in Third impact

There is also the possibility that Rei 1 died during the Rei I and Rei II
episodes, but I really saw no direct evidence that she did. Both Gendou
and Shinji saved her from the entry plugs.
