Well, earlier in the fic, Akane said:

"That's another thing. Our genetic trait is supposed to be
absolutely secret. Our school's principal and our doctor are the 
only ones in the area who know about it."

Tofu and the Principal, knowing about the clan's propensity for
gender-changing, are assuming that Ranma is another of their
clan. (Shimeru?  One's bad with names.)
