I've had some responses on this.  To report:

Nobody seems to know their last names.

ie their last names have never been mentioned in the anime.

However, Gary Kleppe mentioned that Hiroshi (the light haired one, at
least, I believe Hiroshi is the light haired one) is referred to in
the Memorial Book.

Which I have and didn't even think of checking.  -_-

Anyway.  I've checked the Memorial Book and Hiroshi is referred to as
Chapatsu-kun.  I haven't found any of the other three.

I've since done a bit more research and discovered that 'Chapatsu' means
'tea hair'.  This is obviously referring to Hiroshi's light hair
colour.  Whether this is a nickname or his surname or his first name (in
the manga) I've no idea.  Yamaguchi, a valid Japanese surname, means
'mountain mouth', after all.

It's probably best to use this as Hiroshi's last name.

When Gary Kleppe said Hiroshi was referred to in the memorial book, he
said Hiroshi was referred to as 'Chappatsu' (written in English with two
ps).  I don't know where this came from for in the memorial book it's
definitely 'Chapatsu', written in English with only one p.

I guess that for fanfic purposes this means you get to make up Daisuke,
Sayuri, and Yuka's last names.  ^_^
