Depends on the transfer.  there are several levels of high schools in
Japan (lowest being vocationary schools, highest being college prep
schools).  There is a High School Exam and your score on that exam
determines which level school you may enter.  I think it's possible to
transfer to an equivalent level school or a lower level school, but it
is not possible to transfer to a higher level one.

2) Are classes segregated completely by grade level, or are there
classes where upper and lower classmen would both attend?

Yes, in Japan students don't move from class room to class room, the
teachers do.  Your curriculum and schedule determine what classroom you
are put into.  The only class where upper and lower classmen might
attend together is Physical Education, but I'm not sure about that.
