Since Takahashi-megamisama has decined to comment on this (she said
something like "I won't think about it, and neither should you."), it's
up to individual authors to do whatever fits into their stories. Does
the pregnancy prevent the change somehow? Would changing to male form
abort the fetus? Or would the male body be modified to be able to carry
the baby? It's a matter of authors' discretion.

I myself have written one story ("Lost") which took one approach to the
question, and am planning another story that uses an entirely different
approach.
