For the familiar effect which usually
occurs among family members, change the -san
to -chan. i.e. - Oniichan.

Unlesss I'm mistaken, the "o" in the beginning is a "formal o" and can also
be removed - ie, "niisan", "neesan", etc (or "niichan", "neechan" etc).

The possibilities are endless. ^^
