Kyousuke's mother's personal name is Akemi. Her family name is unknown, 
though some have suggested that Kasuga is her family name and that Takeshi 
(Kyousuke's father) took his wife's name. I think I have seen this 
referred to as "muko ni naru". 

As for the year of Kyousuke's and Madoka's birth, it depends on which 
timeline. The manga and KOR:Ano hi ni Kaeritai say 1969. The TV series 
and ShinKOR (both novel and movie) say 1972. Most people accept 1969.
