Both.  No matter how many different interpretations fanfic writers have, 
Sailor Pluto's origins will still remain mysterious because we don't know 
what Naoko Takeuchi (did I spell her name right?) has in mind.

It's the same way all the stuff at http://www.randomc.com/~arromdee/rp/ 
doesn't resolve that what-if because Rumiko Takahashi says she doesn't think 
about it.
