Setsuna is Susan, the Doctor's granddaughter.  Think about it.

I'd think Susan would still be the Doc's granddaughter no matter which
regeneration. She knows it happens; she goes through one herself (Susan
looks nothing like Setsuna). On the other hand, it has been a long time
since they've seen each other. Thus, I'm using the eighth (Paul McGann).
