Yay!  A Sucker!... I mean volunteer!  (though I would have preferred a 
non-author - now everyone's going to blame me when your fanfic is taking too 
long)  ;)

Here's the format I was fleshing out.  Feel free to change it since you're the 
one doing the archive.  If you decide not to do it, please tell me.

Author's Anime/Manga Fanfic Guide Suggested Format

Submission guidelines:
1.  Authors get first crack at writing their own entries.  After a month 
or so, there may be non-author submissions for the sake of 
completeness (these will be marked for identification).
2.  Fanfics must be available at a permanent archive site, including 
the raac archive or the fanfic remailer (other archives to be added as I 
find out about them).  Web pages can also be considered archives 
only if it is author or fanfic series specific (no using someone else's 
web page - though manga specific archive pages may be added as 
above).
3.  Fanfics that have been submitted but not added to an existing 
archive may have entries.  These will be marked and removed after 
three months if it is not added to the archive.

based.  If the fanfic is a crossover, separate titles by dashes.

[MegaXOver]	More than 2 titles
[Original]	No series base

2.  [Fanfic Title] - Title of Fanfic or Fanfic Series.  To keep the size 
of the guide manageable, the author is requested to collect all short 
unrelated fanfics under the title of "[author]'s One-Shots" or 
something similar and use the Series format (say smaller than one of 
Joseph Palmer's "seasons" for lower size limit).

3.  [Fanfic Type] - Applicable fanfic designations.

[Continuation]	If after end of anime/manga
[NextGen]		If focus on descendents
[X-Over]		If crossover between series
[Elseworlds]	If alternate universe
[Darkfic/Sadfic]	If written to depress readers
[Lemon]		If includes descriptive sex
[Poetry]		If poetry format
[Parody]		If parody of original or fanfics
[Lyrics]		If lyrics format
[Side Story]	If concurrent but not part of the main storyline
[Series]		If parts/episodes are complete stories

7.  [Continuity Level] - Level of consistency with original
[Full]		Consistent with series end
[Partial]		Consistent to some defined point
[None]		No continuity, though most facts may be similar
[Manga/Anime]	Consistent with source

8.  [Fanfic Status] - Level of completion
[Complete]	Finished; no further planned episodes
[On-Going]	Still being written (Series only)
[Incomplete]	Still being written (Non-Series only)
[Uncomplete]	Unfinished, no releases within past nine months

9.  [Fanfic Availability] - Archives that have the fanfic

10.  [Description] - Brief description of fanfic, two to three lines at 
most.  Also any awards such as "Ranma Fanfic of Year".  Fanfic 
Series can also add one line episode descriptions for each episode 
using the following format:

[Episode#]. [Episode Title][Additional Qualifiers] [#parts] - 
[Description]  [Author if different]

[Additional Qualifiers] - Any qualifiers for a specific episode that is 
in addition to the fanfic qualifiers
[#parts] - If the episode was released in distinct parts

December's End (10 Episodes) - Ranma gets sick and tells Akane he 
	oves her.
A Day In The Life (3 Episodes) - A partially typical school day from 
	he POV of the three daughters.

I'd suggest replacing this example with your own.
