I STILL do not know HOW to get ahold of the form for sure, but I was
told to ask the Queen's Own fanclub.  Also You MIGHT want to try 

WRITING

or emailing her pubishers (Baen and DAW Books...) for more Information.

I've tried this...actually, I tried asking how one could contact Lackey 
directly.  For reasons that are probably obvious, Baen will not give out 
such information, but will forward mail or email to her.  Their address 
is Baen Books, PO Box 1403, Riverdale NY 10471, and their email is 
jim_baen@baen.com.  I'm planning on sending a letter with a SASE (the 
chances of recieving a reply are probably greater that way).  We'll see 
what happens.
