The details are different, but this plot was also in the TV anime.

But why would he teleport to another universe if he's looking for
Madoka?
I'd think he would be more likely to teleport to her house in HIS
universe, even running on blind instinct.

If not her house, then why not his own house?  At the time of the
incident, he's experiencing the need for safety and most people
associate home with safety.

Hikaru was mostly drawing a blank, although the name sounded

familiar.

"Who's Kazuya?"

Hasn't she met the brat?

She did in the anime for a couple of episodes.  He came close to
breaking her heart by revealing to her that Kyosuke like Madoka more.
Kyosuke preserved the status quo at the time by denying the whole thing
and Hikaru at the time was more than willing to accept it.
